I didn't see any signage on VA 384 earlier today, either at the entrance to the college campus or anywhere along its route through the parking lot.

VA 154 is still signed at I-64, but that's the only sign I found. I followed it, or any road that might be it, through Covington and didn't see another sign. VA154 may as well stay, but on VA154cov all I found were trailblazers for US 60 & US 220, so it probably ought to go. If you decide to keep it, I's suggest realigning it to follow Riverside Ave all the way to US220 since that's the route with the trailblazers and (IIRC) junction sign at the end.

VA 384 sign was still there as of Oct 2016 GMSV - https://goo.gl/maps/6hLMd8kph3C2.  That would be too bad if it is now gone.  It was still a cutout the first time I went over there...

I didn't draft the Virginia State set but my guess was the thought was since VA 154 is posted at I-64 and the gap is only a couple blocks, the whole thing was included.  VA 154 has not been posted within Covington's city limits in at least 20 years (if ever).  It's official route is as represented in the HB though, despite the illogic of it.  I can certainly delete the 154cov file if that is the consensus...

Did you make it to this location at the south end of the unposted piece (Lexington at Riverside)?  Is this awesome US 220 shield still there?

I noticed the sign for the college, but didn't see a shield attached. Not knowing that was the only place it was signed, I didn't think to check more closely, to see if it had been bent back or something. Might be worth further investigation.

Here is the verbatim description of VA 154's creation in April 1962 - "up S. Durant Ave to the north side of Jackson River; thence northwesterly and generally parallel to the Jackson River and the C & O Railroad Belt Line to a connection with Riverside Street at its intersection with North Lexington Ave; thence northeasterly along Riverside Street to North Alleghany Avenue (US 220)"

This description implies that it wasn't supposed to follow Durant (becomes Lexington) all the way to Riverside directly.  But regardless, In Feb 1968 VA 154 was moved to Craig St which created the gap.  The 1975 Alleghany map (see below) showed an intended smooth transition through the gap on a road yet to be built.

The 1962 description does not match what it does on Hickory St either.  I never found a CTB reference to address that.

File submitted to delete 154Cov.

I will wait on 384...I am scheduled to field check this in February.  I am also curious about VA 188 in Clifton Forge since GMSV on US 60 TRUCK shows the 188 shield gone (rest of 188 has no GMSV).
